Rachel Fleming - RN/Hospice Case Manager

Rachel comes to Evergreen with nearly 2 years of hospice experience and is very excited to join a team and culture that believes passionately in patient-centered care. She is an RN who starting off her nursing career in a hospital and was a nurse case manager in home health for nearly 5 years. She found that many of her patients were declining and more appropriate for hospice care than home health and felt a tug on her heart to move into hospice. Besides her love for hospice nursing, Rachel loves God, her family (she’s married to Jeff and they have 2 teen boys, Jacob and Eric), her dog (Noey Joey) and her extended family and friends. “I feel like I’m home at Evergreen and plan on being here for a long time!”

Kaitlin hopes to share her passion for providing comfort and quality at the end of life.

She feels honored to work with such a dedicated and compassionate team, and has found her home away from home with them. When Cory is not at work, she enjoys spending time with her family outdoors or watching movies, as well as watching her three kids participate in their various activities.
